Our Program Can Help You
At no cost to you, CareVio is a care management program that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.
About Us
We provide evidence-based care plans and individualized case management services to help you maintain a healthy pregnancy and reduce the risk of premature delivery.
We Provide:
- Support and guidance throughout pregnancy and the arrival of your new family member.
- Assistance in identifying social needs as well as connecting you to resources to meet those needs.
- Help with overcoming barriers, creating plans for interventions, and achieving a healthy and
successful pregnancy and delivery. - Referrals for community resources, Social Work and/or Pharmacy as needed.
- Support after an Emergency Department visit and/or hospitalization.
This Program Supports You:
- If you use CareVio.
- If you are pregnant with one or more of the following:
- Diabetes/Gestational Diabetes
- Hypertension >140/90
- BMI >40 or <18.5
- Psychiatric Diagnosis
- Substance use Disorder-Alcohol
- Teen Pregnancy <16
- If you have a ChristianaCare primary care physician and Medicaid insurance.
