The goal of the CareVio CTO

The CareVio^® Collaborative Care Transformation Organization (CTO) is a Care Transformation Organization participating in the Maryland Primary Care Program (MDPCP), an initiative of the Maryland Total Cost of Care All-Payer Model through the Centers for Medicare & Medicaid Services (CMS) and the Maryland Department of Health (MDH).

The primary goal of the CTO is to work with individuals and practices to improve health and self-management, reduce health disparities and inequities and enhance connectedness and follow-up with primary care. We aspire to create a sustainable support system by leveraging the health system, community, local, state and federal resources. These strategies will not only improve health outcomes but will reduce unnecessary utilization and prevent avoidable hospital events.
