CareVio offers an array of programs and services to the members served. Five CareVio programs are accredited by the National Committee for Quality Assurance (NCQA). The CareVio programs and services follow a domain structure that was established in 2022 to provide optimal healthcare outcomes as members move along the continuum of care from transitional to longitudinal settings.
If you have a chronic medical condition such as diabetes, congestive heart failure or COPD, your doctor may enlist the help of the award-winning CareVio care coordination team to help you stay on top of all of the moving pieces of your care plan. CareVio can help you track down labwork, schedule appointments, keep up with prescriptions, make sure every member of your health care team is up to date with how you’re doing, and support you through life’s challenges that get in the way of your good health.
