Our Program Can Help You
At no cost to you, CareVio is a care management program that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.
About Us
We provide evidence-based care plans and individualized case management services for parents of children 18 and under to help manage their child’s health and healthcare goals.
We Provide:
- Assistance in understanding your child’s current health problems.
- Help with preventative care testing and immunizations.
- Assistance in identifying the social needs of your child and/or your family as well as connecting you to resources to meet those needs.
- Tools to help you better understand your child’s medication.
- Help with creating individualized goals to overcome barriers along with a self-management action plan.
- Referrals for community resources, Social Work and/or Pharmacist as needed.
- Support after an Emergency Department visit and/or hospitalization.
This Program Supports You:
- If your child is 18 years old or under and uses CareVio.
- If your child has complex medical needs.
- If your child is discharged from Neonatal Intensive Care Unit (NICU).
- If your child visits the Emergency Department or hospital services often.
- If it is recommended by your child’s doctor or CareVio staff.
