Our Program Can Help You
At no cost to you, CareVio is a care management program that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.
About Us
Our mission is to help support you with your health needs, enhance current life skills, and offer tools for self-care. We are here to help you with your medical, social, and behavioral needs and connect you to community support resources.
We Provide:
- In person or virtual help with your healthcare needs.
- A complete assessment by a licensed social worker.
- Partner with your doctor to create intervention plans.
- Assist you in personalizing your social and behavioral goals.
- Support at medical appointments as needed.
- Offer support tools for self-management skills.
- Referrals for community resources and health care services.
This Program Supports You:
- If you use CareVio.
- If you visit the Emergency Department or use hospital services often.
- If you have high hospital bills.
- If you are at risk for hospitalization.
- If recommended by your doctor or CareVio staff.
