What it Is

CareVio®, powered by ChristianaCare, is an innovative, technology driven, care coordination program ideal for employers and health insurance companies who are looking to incorporate a program that adds real value to their program offerings.

Serving nearly 75,000 Medicare beneficiaries and health plan members in the greater Delaware region, CareVio® has been selected by small and large organizations alike. How it Works

Unlike traditional care coordination programs, CareVio® harnesses a real-time technology platform that integrates various patient information – hospitalizations, emergency department visits, physician visits, lab results, x-ray reports, pharmaceutical use and claims data. Clinical experts on the CareVio® team – doctors, nurses, pharmacists, social workers and more – use this data to proactively identify populations most at-risk or in need of attention so issues can be addressed before they escalate.

CareVio® team members support patients across the continuum of care from hospital to home. Outpatient outreach starts as soon as patients return home, with a focus on symptom management, follow-up appointments and access to discharge medications, all while supporting primary care and sub-specialty physicians in the care of their patients.

CareVio® team members educate patients on accessing the most appropriate site for care, helping to minimize unnecessary use of the emergency department. The technology platform identifies patients at highest risk of utilization, allowing the team to customize the intensity of outreach to each patient.

Real-time admission alerts on patients from all emergency departments and hospitals through the regional health information exchanges, along with other health information and claims data, allow the team to interact with hospital clinical and care management teams, helping to avoid unnecessary hospitalizations and ensuring appropriate follow-through on clinical plans.

How this Program Benefits Patients

CareVio® Care Coordinator Mary Jones learned that Mr. Wright never filled his last prescription for his medication. With one phone call, she discovered that his car was in the shop and he had no way of getting to the pharmacy. His CareVio® team, including a nurse care coordinator, social worker and pharmacist, not only helped get him his medications, but worked with his doctor to teach Mr. Wright new ways to more effectively manage his condition through medication, diet and exercise.

Mrs. James drove herself to the emergency department again this weekend seeking help for complications from her high blood pressure. Before she even had a chance to update her Facebook page and let friends and family know she was there, CareVio® sent a real-time alert to her primary care physician, launching a proactive plan to more effectively manage her condition and avoid future costly emergency visits.
