Our Program Can Help You
At no cost to you, CareVio is an innovative care management organization that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.
About Us
Our team includes Medical Directors, Registered Nurses, Pharmacists and Social Workers. We provide case management services to help you have a healthy pregnancy, lower your chance of early delivery and support you after delivery.
We Provide:
- Support and guidance throughout pregnancy and the arrival of your new family member.
- Connections to resources to help you with things like housing, finances, food and transportation.
- Help with creating goals and an action plan that are right for you.
- Education for a healthy and successful pregnancy and delivery.
- Referrals for social work and/or pharmacy as needed.
- Support after an Emergency Department visit and/or hospitalization.
This Program Supports You:
- If you use CareVio.
- If you have Medicaid insurance.
- If you had problems with a previous pregnancy.
- If you are pregnant with one or more of the following:
- Diabetes/Gestational Diabetes
- High Blood Pressure
- Overweight/Under Weight
- Mental Health Needs
- Substance use Disorder
- Teen Pregnancy <16 years old
- Complex Social Needs
