Our Program Can Help You

At no cost to you, CareVio is an innovative care management organization that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.

About Us

Our team includes Medical Directors, Registered Nurses, Pharmacists, Social Workers, Behavioral Health Consultants, and Certified Advanced Alcohol and Drug Counselors. We will help you with taking care of yourself to live a safe and successful life. Our goal is to support you in building life skills and connecting you with the right resources.

We Provide:

In person or virtual help with your medical, mental health, and/or social needs.
Support tools for self-management skills.
A complete assessment by a licensed social worker.
Help with creating goals and an action plan that is right for you.
Partnership with your primary care and/or specialist in your treatment plan.

This Program Supports You:

If you use CareVio.
If recommended by your doctor or CareVio team.
If you visit the Emergency Department often.
If you have health conditions requiring in-person help.
If you have behavioral needs (such as):
- Thoughts of hurting yourself or others
- Substance Use
- Mental Health Needs
