Our Program Can Help You
At no cost to you, CareVio is an innovative care management organization that will work with you and your healthcare team to make it easier for you to take care of yourself and stay well.
About Us
Our team includes Medical Directors, Registered Nurses, Pharmacists and Social Workers. We provide case management services to patients who need a lot of resources and help. We want to support you in your health and achieving your healthcare goals.
We Provide:
- Education to help you understand your current health problems.
- Help with creating goals and an action plan that is right for you.
- Tools to help you better understand your medicine.
- Connections to resources to help with things like housing, finances, food and transportation. Help with preventative care testing and immunizations.
- Referrals for social work and/or pharmacy as needed.
- Referrals for specialty care, palliative care, and/or hospice services as needed.
- Support after an Emergency Department visit or hospitalization.
This Program Supports You:
- If you use CareVio.
- If you are at risk for hospitalization.
- If recommended by your doctor or CareVio team.
- If you have more than one health condition that is difficult to manage.
- If you have any of the following health problems (such as):
- Asthma
- Congestive Heart Failure
- COPD
- Kidney Disease
- Diabetes
- Cancer
